MCMS OTC Medication Form

Due to a situation that was out of our control, one of the forms that is typically sent home on the first day of school could not be distributed. The issue has been rectified and your child will be bringing a medication form home from school today, please complete and return it with your student as soon as possible if you have not done so already. Students will be receiving forms to bring home today. Without this form, your student is unable to receive medication/treatment from the Nurse’s Office. Thank you for your cooperation and understanding.
