The following health information is required for original entry into kindergarten or first grade. Additional information is required and can be obtained form the school office.

Kindergarten Requirements

  1. Health History Form

  2. Immunizations

    • 4 DTP (Diphtheria and Tetanus)

    • 3 Trivalent Oral Polio

    • 2 MMR (measles, mumps and Rubella)

    • 3 Hepatitis B

  3. Physical Examination Report

Other Information which needs to be returned:

  • Private Dental Report

  • Medication Permission/Refusal Form
