On January 31, 2007, the Freshmen (9th grade students) will be given a letter and forms with information about the vaccine program.  Included will be a permission/refusal form and specific information about the immunizations being offered.  Two vaccines will be offered at no cost to the family.

All permission/refusal forms must be returned by February 9, 2007.  The immunizations will be administered sometime in April, 2007.
