The Pennsylvania School Health Law requires all children to have a physical exam completed upon original entry into the school, in the 6th and in 11th grade.
If your child gets a physical for any reason(example: routine "wellness check" physical, amp physical, sport's physical and/or driver's permit) please have the a form completed by your physician. Forms are available at the school physician office and on-line at the MCSD website, click on Nurse.
The completed form must be returned to the Indian Valley High School office during the summer of 2010/2011 school year.
Physical exams may be obtained one year prior to 11th grade, but must be completed by the first day of 12th grade.
It is recommended that a family physician complete the examination report fore the school record, because the physician had good knowledge of your child's medical history.
