Beginning school year 2010/2011-Students IN ALL GRADES need 2 doses
of varicella (chickenpox) vaccine or history of disease. A message about junior
physicals-PA School Health Law requires all children to have a
physical exam completed: If your child gets a physical for any reason please have the form
completed by your physician. The completed form may be returned to
the IVHS office during the summer of 2009/2010 school year.
Physical exams may be obtained one year prior to 11th
grade, but must be completed by the first day of 12th
grade. If your child needs to take any
medications during the school year-prescribed or over-the-counter and
order must be written by the physician. If your child has a food allergy
and accommodations need to made in the cafeteria a written note from a
physician needs to be provided. This only needs to be completed one
time. All form are available in the web site
and in the school office during the summer time.
If your child has already had a physical completed
for a driver's permit and they do not plan to receive a sports physical
contact the school nurse.
