Friendly Reminders

Beginning school year 2010/2011-Students IN ALL GRADES need 2 doses
of varicella (chickenpox) vaccine or history of disease.
-
A message about junior physicals-PA School Health Law requires all
children to have a physical exam completed upon original entry into the
school, in the 6th grade, and in
11th grade.

If your child needs to take any
medications during the school year-prescribed or over-the-counter and
order must be written by the physician.
-
If your child has a food allergy
and accommodations need to made in the cafeteria a written note from a
physician needs to be provided. This only needs to be completed one
time.

and in the school office during the summer time.

*** Attention all Fall Athletes ***
The Athletic Director has been notified that Family
Health Associates (FHA) will not be offering discount physicals
this summer to Lewistown or IV athletes and all athletes will need a
private physical before August 17th to participate in Fall Sports.
