If your child is at a greater risk for a medical emergency while being transported to and from school, please complete all sections of the medical emergency transportation form located under the students/parents tab of the MCSD website under forms OR you may select the link at the bottom of this announcement to view/print the PDF version of the form.

In an attempt to better serve your child in the event of a medical emergency while on school transportation to/from school, we would ask that you complete the form.
